The Road Vehicles (Authorisation of Special Types) (General) Order, 2003 Schedule 5 or Schedule 9 Part 1

(Subject to the prior agreement of each of the authorities to which this notice is sent, it may be used to give notice of additional journeys, different vehicles, routes and destinations)

In pursuance of Part 2 or Part 4 of the above Order, I / we being the operator of the under mentioned vehicle(s) to which the Order applies, hereby give notice that it is my / our intention to use the said vehicle(s) on the roads specified below. The route and Department for Transport Special Order Numbers of VR(1) or BE 16 movements only are:
